1st suggestion for a structure:
the sugested structure is a 5x5 matrix with the states
No CHD
Angina
Myocardial Infarction
Death by CVD
Death by General Mortality
The mortality is computed based on a corrected standard mortality and a CHD/MI specific probability to die with three influences:
sudden death (with no MI or Angina before)
cardiovascular death and Angina (meaning a yearly cycle with an Angina avent before death occurs)
death due to mayocardial infarction
Do we really need five states? I would prefer to see only four states. As death is death and the counting of patients flowing from different directions to death can easily be counted in the specific transitions...
